SMS Consent Form

TABI Clinic offers appointment reminders, general business communication, holiday closures, and feedback by text message. If you wish to receive those messages, we require your written consent. To protect patient privacy and ensure compliance with healthcare communication requirements, participation in our SMS messaging program requires a signed, physical consent form. Consent submitted through this website, by email, or by text message will not be accepted.

Patients, parents, guardians, or authorized representatives who wish to receive text messages from our practice must complete and sign a hard-copy SMS Consent Form and return it to our office. SMS communications will not begin until the signed form has been received and processed.
